BACKGROUND: Papillary fibroelastomas are rare, benign primary cardiac tumors. They are typically found on valvular surfaces, most commonly the aortic valve. In this article, we report a case of papillary fibroelastoma arising from the apex of the left ventricle, a rare and unusual site causing an embolic stroke. We describe a left ventricle fibroelestoma resection with a robotic approach.

METHODS AND RESULTS: 71-year-old man presented with diplopia and weakness: diagnosis of trunk stroke and consequent thrombolysis. Discovery of a small mass in the apex of the left ventricle.

CONCLUSION: Thanks to the help of the Da Vinci robot we were able to perform the excision of a rare tumor for this anatomical position. Robotic surgery has demonstrated, once again, its effectiveness in an intervention that could have been very complicated and invasive if done in a conventional way.
